Helping patients return safely home from hospital
We work with the Integrated Discharge Hub based at Royal Stoke University Hospital to discharge patients safely home. We understand people's changing needs and can put steps in place to address any barriers preventing people from returning home and living independently again.
Who is this service for?
This service is eligible for:
- North Staffordshire residents,
- over 18,
- with low-level support needs.
How to access this service
If you're a patient you'll need to access this service directly through the integrated discharge service based at Royal Stoke University Hospital.

Our helpline and email inbox is responded to between 9am-5pm, Monday-Friday. We also record all phone calls so we can make sure we give everyone the best customer service.
